報名程序 register procedure

安全講習將採線上課程、線上測驗及現場實體考核三階段方式進行，以下為具體流程與說明，考核費用依學生實習工廠申請使用規章為準：
The safety training will be conducted through three stages: online learning, an online assessment, and an on-site practical evaluation. The detailed process is as follows:

── 認證流程 Certification Procedure ──

① 預約登記 Registration
Please provide the following information when making a reservation:
1. 基本資料：系級／學號／姓名／Email
Basic information:
Department / Student ID / Name / Email
2. 可實體考核時段（請以半天為單位）
Available time slots for the on-site practical evaluation (Please select in half-day units)
（上午場為9:00-12:00，下午場為14:00-17:00）
（Morning session: 9:00–12:00，Afternoon session: 14:00–17:00）
② 線上課程 Online Course
完成預約後，將提供線上課程連結，
請點選連結完成課程：
After completing the reservation, an online course link will be provided.

③ 線上測驗 Online Assessment
完成課程後，主動聯絡工廠官方LINE
索取線上測驗連結
After completing the online course, please contact the Factory Official LINE account to request the online assessment link.
線上測驗 Assessment results:
達85分以上者→可於官方LINE預約實體考核時間
Score 85 or above → Eligible to schedule an on-site practical evaluation via the Official LINE account
未85分以上者→於官方LINE索取第二份測驗連結  
Score below 85 → Request a second assessment link via the Official LINE account
※  測驗至多3次，未通過者需於一周後才能再次申請線上測驗
※ A maximum of three attempts is allowed. Students who do not pass after three attempts must wait one week before applying again for another online assessment.
④現場 實體考核 On-site Practical Evaluation
線上測驗通過後，請依通知於預約時段至學生實習工廠，進行現場機台實體考核
Students must attend the Student Practice Workshop at their reserved time slot for the on-site equipment operation evaluation.
實作時間約一小時
The practical evaluation takes approximately one hour.
※ 考核未通過者，需於一周後才可再次預約實體考核
※ Students who do not pass the evaluation must wait one week before making another reservation.
⑤ 開卡 Access Activation
實體考核通過者，簽署完安全切結書後
當天即可完成開卡程序
Students who pass the practical evaluation must sign the Safety Agreement Form. Access authorization will be activated on the same day after completion of the procedure.

本課程相關認證流程、作業方式及實施細節，成大工設學生實習工廠保留最終解釋權，並得視實際執行情形調整之。
The NCKU Department of Industrial Design Student Practice Workshop reserves the final right of interpretation regarding the certification procedures, operational methods, and implementation details. Adjustments may be made when necessary based on actual conditions.

安全講習課程注意事項 Safety Lecture Guide

• 實體考核時間約為1小時，上午場為9:00-12:00，下午場為14:00-17:00，每小時至多3人，實際時間以開課通知為主，請欲參加的同學預留時間。

The on-site practical evaluation takes approximately one hour. The morning session is from 9:00–12:00, and the afternoon session is from 14:00–17:00. Each time slot accommodates up to three students per hour. The exact evaluation time will be confirmed in the official course notification, so students who plan to participate are advised to reserve sufficient time in advance.

• 參與實體考核的同學，遲到超過15分鐘取消資格，並不再受理報名。

Please be punctual. Students who are over 15 minutes late will be disqualified and will not be able to register again. 

• 報名參加實體考核的同學，請於參加課程當天，攜帶學生證至工廠管理室填寫工廠使用安全同意書。

Please bring your student ID on the lecture day and fill out the safety agreement. 

• 若本院師生欲參與安全講習課程，可提前來電詢問，校內分機54305或掃描QR-Code。

College of Planning & Design students and faculty willing to participate in the safety lecture, please call the campus ext. 54305 or scan QR-Code for inquiries in advance.

• 參與實體考核的同學，請穿著工廠規定服裝，並自備口罩、圍裙、眼罩（眼鏡）、長頭髮者需將頭髮紮起、嚴禁拖鞋與涼鞋。

Please wear proper outfits, including face masks, aprons, protective goggles to the safety lecture. Long hair needs to be tied while staying at the workshop. Sandals and slippers are Strictly Forbidden.

• 若因臨時有事無法參與實體考核，請提前與工廠管理員聯絡。如無故未到或當日取消者，將取消優先上課之權利。

Please contact the manager or staff before the lecture if you cannot attend the course. If unexcused absent or temporarily canceled will be deprived of priority.

• 實體考核之材料工廠會視經費考量提供。

Lecture materials will be provided depending on fundings.
